It is that time in the quarter where anyone with a grade below 70% is required to meet with course faculty and complete a progress report form.

If you haven't received an email yet from your instructor and you have a grade below 70%, contact them.   All progress report forms are due back to me no later than Monday February 20th. 

I'm attaching a copy of the form.  If you are failing more than one class you will need to submit a page 2 for each  of those classes.

Note:  This only applies to students who have a grade below 70% in any class.
