Reminder that attendance is documented on a weekly basis and is confirmed based on your submitted time sheet document, which is due no later than Sunday of each week.

If you are absent, please let your clinical faculty and site know ASAP.   All medical excused absences must be accompanied by a physicians note within 5 days or they will be unexcused. 

Any long term illnesses/absences or requests for vacation or leave of absence must additionally go through the program director and registrar's office.
